When it comes to palatal expansion, is faster always better?

In this episode, Renata Nehme speaks with orthodontist Dr. Claudia Pinter about how advances in bone-borne expanders are changing the conversation around rapid and slow maxillary expansion.

They explore:

✨ Why rapid expansion was traditionally used with tooth-borne appliances
✨ How miniscrew anchorage can make slower expansion possible
✨ The differences between tooth-borne, hybrid, and bone-borne expanders
✨ Why CBCT-guided customization and screw placement matter
✨ How slow expansion may support a more gradual remodeling process
✨ The possibility of coordinating expansion with clear aligner treatment
✨ Why retention and bone mineralization require patience
✨ Factors that may contribute to asymmetry, complications, or relapse
✨ When piezo-assisted techniques may be considered
✨ Why appliance selection and activation should be individualized

Renata also shares her experience with adult MARPE, including the initial activation, development of a diastema, changes in her activation schedule, and improvements in breathing and tongue space.

Dr. Pinter explains that the rate of expansion is only one part of the treatment. Patient age, sutural maturation, available bone, appliance design, miniscrew stability, oral hygiene, treatment goals, and retention must all be considered.

She also discusses using customized bone-borne appliances and CBCT imaging to identify stronger areas of palatal bone rather than placing miniscrews according to a prefabricated design. This approach may help reduce unwanted dental movement and the risk of appliance failure.

Dr. Claudia Pinter is a leading expert in aligner technique who specializes in invisible orthodontics with clear aligners. A graduate of the University of Vienna, she is an internationally sought-after speaker renowned for simplifying complex concepts into practical principles that clinicians can apply in everyday practice. She inspires doctors to deepen their understanding of aligner techniques and is known for openly sharing her clinical protocols.

🎧 Dig into the details and discover why successful expansion depends on much more than how quickly the appliance is activated.
