How early should we begin evaluating a child’s jaw development?

In this episode of the Airway Answers Podcast, Nicole Goldfarb welcomes orthodontist and periodontist Dr. Marianna Evans to discuss why waiting for permanent teeth may mean missing an important opportunity to support facial growth, breathing, and periodontal health.

The conversation explores:

✨ Why crowded teeth may indicate an underlying jaw-development problem
✨ How deficient jaw growth can affect nasal breathing and tongue space
✨ Why early intervention may help permanent teeth erupt with better bone and gum support
✨ Clinical signs that can be recognized without relying solely on 3D imaging
✨ Why clinicians should examine the child’s face before looking inside the mouth
✨ How recessed jaws, poor nasal breathing, and dental crowding may be connected
✨ Why visible spacing does not always mean there is enough room for the permanent teeth
✨ How primary teeth can support expansion appliances in young children
✨ The importance of customizing expansion according to dental and chronological age

Dr. Evans also introduces 4D Morphotropic Orthodontics, an approach that considers jaw development in three dimensions while recognizing age and timing as the fourth dimension. Rather than focusing only on tooth alignment, the goal is to support skeletal development, position the teeth appropriately within the bone, and promote healthier function.

Dr. Marianna Evans received her dental degree and specialty training at the University of Pennsylvania, where she also serves as an adjunct professor in the Sleep Surgery Department. She is the founder of the OrthoPerio Institute and conducts research on the relationship between orthodontics, periodontal health, airway function, and systemic health.

This episode offers a compelling perspective for orthodontists, pediatric dentists, periodontists, myofunctional therapists, and other clinicians involved in early childhood care. When jaw deficiency is identified early, treatment may have the potential to influence far more than the appearance of a child’s smile.

🎧 Explore why timing matters and how earlier collaboration may help change a child’s developmental path.
